Local application of heparin ointment as a trigger factor for the heparin-induced thrombocytopenia

Heparin-induced thrombocytopenia (GIT) is a paradoxical reaction of platelets to heparin (their aggregation and destruction), potentially dangerous by the development venous arterial thrombosis, less often bleeding. There are serious problems timely diagnosis this complication. They due both insufficient awareness specialists presence negative psychological attitude. The article presents clinical case treatment 57-year-old patient who was prescribed local application ointment for catheter thrombosis superficial veins left upper limb. After 8 days, developed instrumental picture occlusive right brachial vein. enoxaparin 0.6 x 2 times day subcutaneously. 4 days since appointment enoxaparin, sharp deterioration in patient’s condition: lower paraplegia with impaired pelvic organ function developed. decrease platelet levels more than 49x109/l. probability developing GIT on «4T» scale points (high GIT). Despite treatment, condition progressively worsened infrarenal aorta, common iliac arteries, massive trunk thromboembolism, segmental subsegmental branches pulmonary artery, intracerebral hemorrhage breakthrough into ventricular system. Conclusion: heparin-induced thrombocytopenia, rare but extremely complication observed patients receiving heparin. key preventing catastrophic consequences awareness, alertness clinicians use modern algorithms GIT.
